HOPS Yearly Registration 2016-2017
With the start of the school year it is time to renew your registration with HOPS. All athletes and volunteers must complete a yearly form prior to participating in any HOPS activity. Athletes will complete a yearly registration form and volunteers will complete a yearly disclosure statement. As we did last year, this year all forms are required to be completed on-line. The forms are located on the HOPS website via the registration tab or by using the links below. This form only needs to be completed once for the 2016-2017 school year.
Registration will be a little longer this year. New insurance regulations requires all registrants to review, understand and acknowledge concussion facts and protocols. This is included in the registration process and you are required to acknowledge that you read, understand and will comply with this information in order to complete registration.
